Preschool Application Forms

We are currently accepting children for our WAIT LIST, as we are fully enrolled for fall 2008. Openings do occur, and families will be notified as soon as an opening exists.

A preschool application form (Microsoft Word format or Acrobat format) and information regarding Maryland State vaccination requirements (Acrobat format) are available for downloading.

Download the 'Official" Immunization Certification Form for your pediatrition to complete (not due to the preschool until orientation in October 2008, but don't wait too long for to make your doctor appointment!)

1. You may email the form back to the lab school (click here) as a file attachment. Be sure to send in your twenty five dollar deposit to the address below. Please make checks payable to: QOHS.

2. Print the application and mail the completed form along with your twenty five dollar deposit to:
